  • Date of birth
  • Place of birth – city and state if known
  • Father's full legal name
  • Mother's full legal name including maiden
  • Occupation – company name and position
  • Veteran's discharge papers (DD-214), if applicable
  • Highest education – name of schools and graduation dates if available
  • Names and relationships of survivors
  • List of Club/Organization memberships, Awards/Achievements
  • Recent photo for hairdresser/barber
  • Full set of clothes including undergarment if choosing a traditional service
